Reports To: Assigned Masters Level IHT Direct Supervisor

A mentor is a core member of the Stabilization treatment team. He/she works very closely with the Family and individual therapists to implement treatment goals. Will Safety Plan will families.

• Provide mentoring/case management services to clients

• Assist with developing, monitoring and implementing an appropriate treatment plan with measurable outcomes
  

• Documentation of all contacts with client and collaterals
  

• Attend team/personnel meeting/s and supervisions
  

• Ensure integration of all program services with regard to client
  

• Monitor and document client progress
  

• Accurate, timely, thorough completion of any/all documentation related to client participation in treatment
  

• Attend relevant meeting/activities pertaining to assigned easels
  

• Collaborate closely and effectively with all other members of the treatment team
  

• Participate in quality improvement and quality assurances projects related to the program
  

• Participate in program planning and expansion activities
  

• Participate in curriculum development, implementation, documentation and enhancement
  

• Consistent delivery of high quality, culturally competent, and appropriate mental health treatment to assigned case/s
  

• Supply information to individual therapist for monthly and other reports

Bachelor's degree in a mental health or human services field is strongly preferred.

Experience working with mental, behavioral, and emotional disorders required. Ability to work as an integral member of a multidisciplinary team is important. The ability to work competently and effectively with clients and other clients of color is a must. Must have excellent speaking and writing skills.
